Paper-Based Collection Instruments

We recognize that accuracy of any paper-based data collection effort depends on the quality of the form design, printing, distribution, and data processing methods. Customized, paper-based instruments, such as reporting forms and surveys, are designed to collect information from key stakeholders. Our team works closely with each customer to develop custom forms with user-friendly layouts that aim to maximize response rates and data reliability. Forms are designed in-house and include a variety of data capture fields.

In order to produce forms of utmost precision, we subcontract printing services to industry leaders known for upholding high-quality printing standards and meeting rigorous delivery deadlines, while maintaining strict confidentiality. Our team coordinates the distribution and return of all pertinent materials. Since delivery methods may significantly influence response rates, it is important to facilitate the process for respondents in order to maximize the quantity of data collected. We strive to provide systematic handling and timely delivery of materials.

At Piedra Data Services, we stand by our commitment to provide the most accurate data to our clients. Completed paper-based forms are processed using state-of-the-art high speed scanners and a sophisticated information capture system. The system reads Optical Mark Reader (OMR) data, barcodes, and hand and machine print. To satisfy the highest standards of data integrity, data validation and cleansing routines are built into the system and highly-trained verifiers review records flagged by the system.

Web-Based Collection Instruments

At Piedra Data Services, we offer comprehensive online tools that aim to incorporate the inherently dynamic features available to software-based systems. Web-based data collection solutions are programmed to enhance ease of use and flexibility, and add wide-ranging functionality to traditional paper-based forms such as password-protected access, instructions and definitions applied to individual questions (i.e., making it easier to provide question-specific assistance), and skip patterns integrated into Web forms, allowing for unique items to be displayed to subsets of respondents. Data collected via Web forms are stored in a secure database and evaluated using the same rigorous data verification processes applied to data gathered from paper-based instruments.
